• Obtain the Funds' prospectuses and read them carefully.
• Download a Nationwide Funds account application, read it carefully and follow all the directions. Other forms may be required; please consult the application, for complete instructions. Within the application you may select to have funds transferred from another custodian by completing the authorization form.
• Return a completed application and all necessary forms along with payment or payment instructions to one of the following addresses.
